What information do I collect from you?

I collect your name, address, home and mobile telephone numbers, email address, occupation, GP contact details, and information about your state of health.

 

Why do I collect this information?

Your personal details allow me to ensure your records are not mixed up with anyone of a similar name.

Your contact details enable me to contact you about appointments (to arrange, re-arrange or confirm them).

Your health and GP details allow me to ensure I deliver the best treatment for your needs, and to contact your GP should the need arise during an appointment.

 

Who might I share this information with?

Your information is not shared with anyone.

 

What do I do with your information?

Your information is held on paper files, which are held at my trading address. Your name and mobile telephone number is also, if authorised by you, held on my own mobile telephone to enable me to recognise you as a caller, and to make outgoing calls to you.

 

How long do I keep hold of your information?

Records are kept whilst you are a client, and for up to 7 years afterwards. You have the right to request that I delete/securely destroy your information at any time after you stop being a client.

 

How can you access the information I hold about you?

You may ask for a copy of the information I hold about you, and a copy of my Privacy Notice, at any time.
